The Families in Transition (FIT) program is a specialized
service for children who may be at high risk for out of home placement,
are in foster care, or need extra support after transitioning
back into their homes after foster care. Our clinicians provide
a combination of therapy and case management to help support children
up to age 21 and their families (including foster families) so
they can heal from traumas such as abuse, loss, domestic violence
and family separations. Many of these children display symptoms
of post-traumatic stress disorder, depression, anxiety or behavior
problems that result in difficulties at school and/or at home.
The unique nature of our program allows clinicians to provide
services, including therapy, in homes, schools, community settings,
or one of our three offices in Alameda County (Oakland, Hayward,
Fremont). This flexibility lets us work with families who might
otherwise not receive services because of transportation or mobility
limitations. Since we provide a combination of services, we often
can be the link between families and the various other agencies
that might also be involved in their lives. Each clinician has
a small caseload, thus we can devote more time to each client
and provide more comprehensive services. With some families we
use a team approach, with more than one therapist working with
the same family in a collaborative fashion.
The Families in Transition team is comprised of Master's level
clinicians trained in either social work or marriage and family
therapy. They receive extensive supervision from licensed clinicians
and on-going training from our in-house training program to keep
up to date on treatment models for the specific issues our clients
face
